What a specialist assessment actually involves

Most patients expect the assessment to begin and end with an imaging referral. In practice, the conversation and the hands-on examination carry equal weight.

The specialist first takes a structured history: when symptoms started, whether onset was sudden or gradual, which movements provoke or ease pain, and what treatment has already been tried. This shapes the clinical picture before any scan is considered.

Physical examination then tests shoulder range of motion, specific provocative tests for impingement, and manual muscle strength. The pattern of findings — which movements are limited, whether weakness is present, and where it localises — guides differential diagnosis alongside imaging.

When imaging is ordered, ultrasound is often the preferred first choice for rotator cuff assessment. MRI becomes useful when deeper anatomy needs to be assessed, when the full extent of a potential tear is unclear, or when associated joint pathology is suspected.

The two modalities answer different questions, and neither substitutes for clinical examination. A tendon change on a scan is one piece of evidence: it is read alongside what the history and physical tests have already revealed, not in isolation from them.

The treatment pathway from conservative care to surgery

For supraspinatus tendinopathy without structural failure, structured physiotherapy is the default starting point. This typically centres on a progressive loading programme — including eccentric exercises targeting the supraspinatus and the wider cuff — combined with activity modification to reduce provocative load while tissue tolerance is rebuilt.

Where physiotherapy plateaus, or where pain is severe enough to impede rehabilitation, a guided injection may be considered as a bridging measure. Corticosteroid injections are commonly used to settle inflammation and allow loading to resume. Platelet-rich plasma (PRP) — which uses concentrated growth factors derived from the patient's own blood to support tendon repair — is an option some clinicians offer at this stage. Evidence for PRP in rotator cuff tendinopathy remains inconsistent, however, and whether it offers a meaningful advantage over other approaches is a conversation to have with the treating specialist rather than an assumption built into the pathway.

Beyond standard injection support, a more selective group of biological or regenerative augmentation techniques may be considered in tendinopathic presentations where useful tendon continuity is confirmed on assessment. These are not appropriate for complete tears or tissue that has detached from bone; structural imaging and clinical examination determine whether the tendon architecture allows them.

Surgical repair enters the picture when structural failure is confirmed, when conservative pathways have been adequately pursued without sufficient improvement, or when tear size is large enough that delay would compromise the repair. This decision sits with an orthopaedic shoulder surgeon and is shaped by both the imaging findings and what the patient needs to return to — work demands, sport, or daily function.
