When Shoulder Pain Points to a Rotator Cuff Tear
What a rotator cuff tear actually feels like
That ache deep in the outer upper arm when you reach for a high shelf, the sharp catch when pulling on a coat, the night disturbed by lying on the affected side — these are the hallmark complaints of rotator cuff trouble. The rotator cuff is a group of four muscles and their tendons that wrap around the top of the upper arm bone, holding the shoulder joint stable and powering its full range of movement. A tear in any one of them can produce this familiar cluster of pain and weakness.
How a tear begins matters. Some arrive suddenly — after a fall onto an outstretched arm, a heavy lift that goes wrong, or a forceful wrench. Others develop silently over months or years, often with no single memorable moment, gradually eroding tissue that has been worn by repetitive load or age-related change. Recognising which pattern fits your own history helps clarify the urgency of what happens next.
It is also worth knowing that MRI and ultrasound scans frequently detect partial signal changes or small tears in adults over 50 who have no shoulder pain at all. A scan finding on its own is not a diagnosis. Equally, some large tears cause surprisingly little discomfort, while some small ones produce severe limitation. Symptom severity and structural size do not reliably track each other — which is why symptoms and daily function, rather than imaging alone, drive clinical decisions.
Red flags that need same-day attention
Some shoulder symptoms cannot wait for a GP appointment. If any of the following apply, seek same-day help — either through A&E or an urgent orthopaedic assessment line:
- Sudden, severe shoulder pain following a fall, collision, or forceful wrench
- Complete inability to move the arm — not just painful movement, but no active lift at all
- Visible deformity of the shoulder joint, or one shoulder sitting markedly differently from the other
- Gross swelling that develops rapidly after an injury
An arm that hangs entirely limp and cannot be raised even a few centimetres after a traumatic event — a presentation clinicians call pseudoparalysis — is a potential surgical emergency. It may indicate an acute full-thickness tear of one or more tendons, a dislocated shoulder, or a fracture. Delay in any of these scenarios risks worsening outcomes.
A separate emergency applies when shoulder pain is accompanied by fever, skin redness over the joint, or feeling systemically unwell. This combination raises the possibility of septic arthritis, which requires urgent A&E assessment and a different clinical pathway from orthopaedic referral.
Everything that follows in this article addresses the far more common picture: shoulder pain that has developed gradually, or that is troublesome but not acute — and how to decide when and where to seek further assessment.
Starting with your GP or a physiotherapist
For most people, the right first step is straightforward: if shoulder pain has not improved after roughly two weeks, or is making it genuinely difficult to move the arm despite simple pain relief and rest, that is a clear prompt to seek help — but not necessarily an emergency appointment.
In many NHS areas, a GP visit is not the only entry point. Community musculoskeletal (MSK) physiotherapy services accept self-referrals directly from patients, without a GP letter. This is often the faster route for a non-acute presentation and is worth checking for your local area before assuming you need to queue for an appointment elsewhere.
Once assessed, first-line management typically centres on structured physiotherapy: exercises targeting rotator cuff strengthening and scapular control, combined with activity modification and appropriate pain relief. A trial of around six to eight weeks along these lines is the clinical standard — not an arbitrary waiting period. Natural history data and evidence-based guidelines support the view that many partial-thickness tears, and a meaningful proportion of degenerative full-thickness tears, respond well to structured rehabilitation, with real gains in pain and function.
If pain is severe enough to prevent engagement with physiotherapy exercises, a corticosteroid injection may be considered within this tier — its purpose is to reduce acute inflammation sufficiently to allow rehabilitation to begin, rather than to serve as a treatment in itself.
When pain and function do not improve after an adequate conservative trial, that is the point at which specialist orthopaedic assessment becomes appropriate — covered in the next section.
When to escalate to an orthopaedic specialist
Three clinical signals, individually or in combination, shift the decision towards specialist orthopaedic review.
Rehabilitation has not produced meaningful improvement. As the previous section noted, a structured physiotherapy trial is the appropriate starting point — but it has a defined endpoint. When pain and function remain substantially unchanged after adequate rehabilitation, continuing the same approach is unlikely to alter the trajectory. Orthopaedic assessment at this stage allows a clinician to evaluate what the conservative programme cannot reveal: tear size, muscle quality, and whether a more active approach is warranted.
Imaging reveals a tear large enough to carry a meaningful deterioration risk. Natural history data show that a rotator cuff tear with a mediolateral diameter of 20 mm or more is associated with severe fatty infiltration of the muscle belly in roughly 80% of cases when managed non-operatively over time (odds ratio 19.0; p<0.001). An anteroposterior diameter of 17 mm or more carries a progression rate of around 73%. These are not self-diagnostic cutoffs — a patient cannot measure their own tear. They illustrate the kind of quantitative information that specialist assessment produces, and why tear dimensions matter beyond symptoms alone. In plain terms: a tear of this size means the muscle behind it is likely to deteriorate without intervention.
Weakness that goes beyond pain limitation. A shoulder that hurts to raise is different from one that cannot be raised at all. When this inability develops gradually rather than after a single traumatic event, it often signals a massive tear in which the force-generating capacity of the cuff is critically compromised. This pattern warrants prompt specialist review.
For younger or more active patients with a confirmed full-thickness tear, the time horizon is a specific consideration. Fatty infiltration of the muscle belly, once established, cannot be reversed surgically. Evidence and clinical experience suggest that delay beyond roughly six to twelve months for large tears risks muscle-quality changes that limit what repair can achieve — a consequence that weighs differently at 45 than at 72.
What an orthopaedic assessment involves
Arriving at a specialist appointment can feel daunting if you are unsure what the clinician is actually trying to establish. In practice, the consultation builds a picture in layers.
It begins with a structured clinical history: when the pain started, whether it followed a specific incident or crept on gradually, how it has changed over time, and what has already been tried. This is followed by a physical examination — measuring how far the arm moves actively and passively, testing which specific muscle groups are producing force, and applying targeted provocation tests designed to stress the individual tendons of the rotator cuff and the subacromial space.
Imaging is then interpreted in that clinical context. Ultrasound is frequently the first-line modality: it is accessible in many primary care and MSK settings, and evidence indicates sensitivity in the range of 81–95% with specificity approaching 100% for full-thickness tears. Where further detail is needed, MRI is the gold standard. It grades tear dimensions, assesses how far the torn tendon has retracted, and — critically — evaluates fatty infiltration of the muscle belly, the factor that most directly influences what surgery can and cannot achieve.
The important qualifier is that a scan finding is not a verdict on its own. A partial-thickness tear identified on MRI in someone whose strength is recovering and whose pain is settling is managed quite differently from the same imaging finding in a patient with progressive weakness. Both imaging and symptoms are weighed together.
The specialist's aim across a first assessment is to stage the tear accurately and match it to a management pathway — a process that may need more than one visit, and that rarely ends with a simple 'operate or not' answer in the room.
Treatment options from conservative care to surgery
The pathway from first symptoms to definitive management follows a clear sequence, though the appropriate stopping point differs for each patient.
Conservative management — structured physiotherapy, activity modification, and analgesia — remains the right starting point for most partial-thickness tears and many degenerative full-thickness tears, particularly in lower-demand patients. Where pain is severe enough to obstruct rehabilitation, a corticosteroid injection may be used to dampen inflammation and restore the window for exercise; its role is as an enabler of physiotherapy, not a treatment in its own right. PRP (platelet-rich plasma) injection is increasingly used as a biologic adjunct in partial tears and tendinopathy, with the aim of supporting tendon repair biology, though it similarly works alongside rehabilitation rather than replacing it.
Surgical repair becomes the appropriate conversation when conservative management has run its course without adequate recovery. A 2024 prospective study found that patients who underwent repair of degenerative rotator cuff tears achieved substantially lower pain scores (VAS 0 versus 3.5) and higher composite function scores (ASES 95 versus 65.8) compared with those who continued non-operative care after pain developed — a meaningful difference that supports surgery as a genuine step-change rather than a marginal gain.
Surgical candidacy is not a straightforward tick-box: tear dimensions, the degree of fatty infiltration in the muscle belly, how far the tendon has retracted, patient age, activity level, and tissue quality all influence both what is technically possible and what is likely to last.
For massive, irreparable tears in older patients who also have glenohumeral arthritis, reverse shoulder arthroplasty is the established surgical pathway, producing reliable improvements in pain and overhead function. Younger patients without arthritis are typically offered joint-preserving alternatives — partial repair, tendon transfer, or superior capsular reconstruction — which avoid replacing the joint whilst stabilising the shoulder. Determining which of these paths applies requires specialist staging; it is not a decision that imaging alone can answer.
