Supraspinatus tendinopathy is a load-related condition affecting one of the four rotator cuff tendons that stabilise and move the shoulder. Degeneration or irritation of the tendon — typically at or near its insertion on the upper arm bone — produces pain at the top and outer aspect of the shoulder, most noticeable during overhead reaching, lifting, or lying on the affected side at night.

Imaging findings alone are not a verdict: mild thickening or signal change on a scan is common and can be present without meaningful symptoms. A specialist uses imaging as one input alongside clinical history and examination, not as a standalone diagnosis.

Approximately 47% of patients with rotator cuff tendinopathy recover by around seven months. That figure is not a reason for pessimism — it reflects that a meaningful proportion will need to progress along the stepped pathway into sports medicine or injection support. Early physiotherapy remains the correct foundation regardless, and recovery is more closely associated with baseline activity level and general health status than with symptom severity at the outset.

Sports medicine physician: bridging conservative and surgical care

When six to twelve weeks of structured physiotherapy has not brought adequate symptom control, a sports medicine physician becomes the logical next specialist — not to replace the rehabilitation already underway, but to extend what conservative care can offer.

Diagnostic precision first

Injectable support

Where confirmed tendinopathy persists despite physiotherapy, subacromial injection can reduce pain enough to allow active rehabilitation to progress. Corticosteroid injection works quickly — typically providing relief over approximately three to six weeks — but repeated use carries a risk of tendon tissue weakening, so it is generally considered a short-term enabler rather than a treatment in its own right.

For more persistent cases, platelet-rich plasma (PRP) and ultrasound-guided hyaluronic acid are evidence-based alternatives. In a three-arm RCT of 60 supraspinatus tendinopathy patients, PRP produced the most favourable short-term improvements in pain and function; separately, three weekly hyaluronic acid injections have shown statistically significant gains sustained through twelve months of follow-up. These results are directional rather than definitive, however — most of the underlying trial populations use broad rotator cuff inclusion criteria rather than supraspinatus-specific samples, and no single injectable stands out as superior across all outcomes and timepoints. The appropriate choice depends on symptom duration, structural findings, and what the patient has already tried.

Orthopaedic surgeon: when structural repair becomes necessary

Surgery is not the natural endpoint for supraspinatus tendinopathy. Most patients manage the condition — fully or sufficiently — through the physiotherapy and sports medicine stages described above, and an orthopaedic referral is appropriate only when defined structural or pathway-failure criteria are met.

The clearest triggers for referral are:

  • Failure of conservative management after three to six months of structured physiotherapy and, where appropriate, injection support. AAOS guidance acknowledges that in some presentations this threshold extends to twelve months before surgery becomes the primary conversation.
  • A confirmed large tear — generally accepted as greater than 3 cm on imaging — where the structural loss is too great for rehabilitation alone to restore function.
  • Complete tendon detachment or marked retraction identified on MRI, indicating that the tissue is no longer in a position from which loading and exercise can drive recovery.
  • Significant weakness — including inability to raise the arm overhead — that persists despite conservative measures and is consistent with structural rather than pain-inhibited failure.

Where onset was sudden and traumatic rather than gradual, the same structural signals apply: prompt imaging rather than a waiting period is warranted to determine whether a complete rupture has occurred.

Once any of these findings are confirmed, the clinical problem shifts from tendon rehabilitation to mechanical repair — a distinction that conservative treatment cannot bridge. An orthopaedic consultation at this stage clarifies whether repair is both structurally feasible and clinically indicated; for some patients with partial tears, it concludes that modified conservative management remains appropriate rather than an operation.
